: All incident reports were screened to remove all identifiable data prior to receipt by the researchers. Given the anonymised nature of the dataset, no formal ethical approval was required for this study. There was no patient involvement. The study protocol was reviewed by NHS Improvement as part of the data-sharing agreement (ref. 5250). Incident reports used for the study are available, by application, from the NRLS and LFPSE. The ID’s of all reports used for the study can be provided upon reasonable request.
