: The study was approved by the Ethical Committee for Research of Federal University of Mato Grosso do Sul (CAAE: 43289221.5.0000.0021). We obtained local IRB approval to waive consent. This was possible because the anonymized data bases from the surveillance system were sent to us only after the linkage and did not allow identification of the study participants. The second cohort was selected from within the surveillance data bases described, and did not involve any further prospective follow-up or data collection.
