: This study was performed after approval by the Institutional Research Board of the Franciscus Gasthuis en Vlietland, Rotterdam, the Netherlands. This research was declared as outside the scope of the Medical Research Involving Human Subjects Act, because routinely collected health care data were used after pseudonymization. This waives the requirement to obtain informed consent. The data from Star-shl was provided anonymously, and as such, the requirement to obtain informed consent was waived.
