: Ethical approval has not been sought for these analyses since the work has been carried out as part of the surveillance of infectious diseases at the national public health institute covered by the national infection control act and regulation on notification system for infectious diseases. The ethics committee/scientific department at the local hospital, Levanger Hospital, has nevertheless been consulted and approval has been given to publish the results in current form.
