: Both the PRIMOS study and the follow-up study were approved by the Regional Ethical Review Board in Stockholm (Ref. Nos. 145/98, 2007/188 − 31/3 and 2011/1743-32) and the Radiation Protection Committee at Karolinska University Hospital. Informed consent was obtained from all participants before enrolment at baseline. The study was performed in accordance with the ethical standards of the Declaration of Helsinki.
