: This study did not require ethical approval. The analyses were performed using a pre-existing claims dataset created as part of the routine administrative activities of a statutory health insurance provider. Its scientific use is regulated by German law in the German Civil Code “Bürgerliches Gesetzbuch”. The data protection officer of the Local Statutory Health Insurance of Lower Saxony-AOK Niedersachsen (Germany) has given permission for this study to use the data for scientific purposes. The study was conducted in accordance with the Declaration of Helsinki.
