: Ethical approval for this study was obtained on September 24, 2020 (ID number: 2020–03264) from the Swedish Ethical Review Authority, which also approved the sending of the reminder mails to the participants on June 29, 2021 (ID number: 2021–03220). All authors have access to the identification key to the pseudo-anonymized participants’ data. Informed written consent was obtained from all participants prior to the data collection. Those who did not attend the onsite clinical evaluations but completed the questionnaires at home and returned them to us were considered to have given informed consent. The study was officially registered at “FoU i Sverige” (researchweb.org) on May 28, 2020 (ID number: 274477).
