: Ethical clearance was obtained from Injbara University College of Medicine and health science department of public health. The study was conducted based on the ethical standards of the Declaration of Helsinki. Then officials at different levels in the study area were communicated through letters from Injibara University, college of Medicine and health science, department of public health. Letters of permission was presented to west Gojjam Zone hospitals. Written informed consent was obtained from each study subject prior to the interview after the purpose of the study was explained to respondent. Coding was used to eliminate names and other personal identification of respondents throughout the study process to ensure participant confidentiality.
