: The study was performed in accordance with the principles of Good Clinical Practice, the Declaration of Helsinki, and UK clinical trial regulations and was approved by an independent ethics committee (Wales REC3 committee, UK—Ref. 14/WA/1219). A trial steering committee supervised the study conduct, and an independent data review committee reviewed accumulating accrual, safety, and treatment data at regular intervals, however, there were no formal stopping guidelines. Patients were required to give informed consent prior to undergoing any trial-specific procedure.
