Ethics approval and consent to participate
: The Proportionate Review Sub-committee of the West Midlands - Solihull Research Ethics Committee reviewed the study application on 16 November 2016 and gave a favourable opinion (REC Reference: 16/WM/0486). HRA Approval was granted on the 23rd of November (IRAS ID 217577). The REC committee determined that individual patient consent would not be required for the use of routinely collected data for this study. This study was performed in accordance with the Declaration of Helsinki.
: This study used routinely collected hospital data kept at The Christie NHS Foundation Trust. The approvals initially received for this study did not allow for the sharing of data, so the data used for the analysis in this study is not currently available.
