: This study was reviewed and approved by the Tanzanian National Institute for Medical Research Ethics Committee, the Kilimanjaro Christian Medical Centre Ethics Committee, and the London School of Hygiene & Tropical Medicine Ethics Committee. It adhered to the tenets of the Declaration of Helsinki. A staff member explained the nature of the study in detail in either Swahili or Maasai language. There was an opportunity to discuss and ask questions. Finally, if the parent or guardian agreed to allow the child to be enroled into the study, this was documented on a consent form in Kiswahili, and witnessed by a third person.
