: Prior to conducting the National Family Health Survey, ethical approval was obtained by the International Institute for Population Sciences from an independent ethics review committee constituted by the Ministry of Health and Family Welfare, Government of India. At the official website of the DHS programme-ExternalRef removed, NFHS-4 datasets are available to the public for use by researchers with all participant identifiers removed, thus this study was exempted from an additional ethics committee review.
