: Ethical approval is not required for retrospective register-based studies in Denmark. However, this study was approved by the Danish Patient Safety Authority (case number:3-3013-2795/1, reference EMGW) and The Capital Region of Denmark (Region Hovedstaden), the institution responsible for the data, (approval number: P-2019-191). Additionally, researchers in Denmark are, under certain conditions, allowed to handle sensitive data. However, to protect personal data all analyses were carried out on a secure server at Statistics Denmark, where all data is pseudonymized, and no results were reported for groups with less than 4 individuals. All authors certify that the study was performed in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards.
