Ethics approval and consent to participate: The study was approved by the Regional Ethical Review Board in Uppsala (Regionala etikprövningsnämnden I Uppsala), an independent national authority under the Swedish Ministry of Education and Research, hosted by Uppsala University (reference number 2015/488). Clinical data were collected from electronic health records and the local transplantation database at Uppsala University Hospital. Written informed consent was obtained from all participants. The study was conducted in accordance with the Declaration of Helsinki and its later amendments.
