: This was an exploratory, randomized, double-bind, controlled study conducted between June 2011 and April 2013 in Singapore and Thailand. All participating centres obtained approval of their independent local Ethical Review Board (SingHealth Centralised Institutional Review Board in Singapore (reference number is CIRB Ref: 2010/832/E) and Institutional Review Board, Faculty of Medicine, Chulalongkorn University in Thailand (reference number is IRB No. 437/55)).
