Ethics approval and consent to participate
: The questionnaire study was reported to the Ethics Committee in the Capital Region of Denmark, which concluded that no ethical approval was needed (reference number: H-17002489). The study was approved by the Danish Data Protection Agency (J. no. 2007-58-0015; Local j. no. GEH-2014-013; I-suite no. 0273) and approval to obtain current addresses on the target population was granted by the Danish Health Data Authority (FSEID-2420). All participants in focus group interviews, cognitive interviews and pilot test were informed about the aim of the study and were assured that participation was voluntary, and the results would remain anonymous. We obtained written consent from all focus group participants and oral consent from all cognitive interview participants. Pilot test participants were informed that responding to the questionnaire would be considered providing consent.
