Ethics approval and consent to participate
: Ethics approval was applied for and granted by the National Institute of Health Sciences Ethics Review Committee prior to commencement of the study (d.22/04/2014). The National Institute of Health Sciences Ethics Review Committee is a Sri Lankan ethics committee. All participants provided informed verbal consent to participate in the survey before any measurements were obtained. As this is a low risk study, written consent to participate was considered unnecessary. The ethics application stated that consent would be obtained verbally.
