: Kitava: Participants gave informed verbal consent instead of written consent due to the participants being illiterate or semi-literate. This procedure and the study protocol were approved by the Research Ethical Committee of the Medical Faculty at Lund University (LU 246-96), the Medical Research Advisory Committee of Papua New Guinea, other Papua New Guinea national and provincial bodies and at the community level by the inhabitants and their tribal leaders. Sweden: Participants gave informed verbal consent instead of written consent due to the informal data collection used in a community study such as the Skara Population Study. This procedure and the study protocol were approved by the ethics committee of the Medical Faculty at Göteborg University (GU 343-93).
