: The ethics committee in both the participating centers approved the study protocol. The study was conducted according to the requirements of the Declaration of Helsinki; written informed consent was obtained from the next of kin or a legal surrogate before randomization, and from each patient who regained mental capacity, according to local ethical approval. All methods were carried out in accordance with relevant guidelines and regulations. All experimental protocols were approved by The Ethics Committee of Central Denmark Region on 2 January 2011 (journal number 20110022).
