: Written informed consent was provided by all women and their children / grandchildren prior to their inclusion in the study. Trained researchers who spoke the participant’s home language explained the study and all participants provided written informed consent prior to taking part. For children, the mother of the child provided written consent that her child may take part in the study, with children older than 7 years additionally providing written assent to take part. The study was conducted according to the principles of the Declaration of Helsinki [] and the Human Research Ethics Committee (Medical) of the University of the Witwatersrand approved the protocol with ethical clearance number M190263.
