Ethical approval for the trial was granted by the Muhimbili University of Health and Allied Sciences’ (MUHAS) Senate Research and Publications Committee (reference number DA.282/298/01 /C) and the National Health Research Ethics Committee at the Tanzania National Institute for Medical Research (NIMR) (reference number NIMR/HQ/R.8a/Vol. IX/3001). Permission to conduct the trial was sought from and granted by the respective hospitals’ administration. Each participant gave written informed consent before being enrolled in the trial. For illiterate participants, informed consent to participate was taken from treatment supporter of illiterate participant. The study participants were given a study-specific number to conceal their identity to maintain confidentiality. No participants’ names were used in the study.
