The linkage of individual-level data from multiple national registers was approved by the Ethical Review Board, Lund (dnr 2018-349) and the Swedish Ethical Review Authority (dnr 2019-01260). Informed consent is not required for secondary use of anonymized register data under European Union General Data Protection Regulation, and therefore the need for consent to participate was waived by the aforementioned ethical review boards.
