: Ethical clearance was obtained from the IRB of the UOG and the Department of Clinical Pharmacy with reference number SOPs/278/2023). Permission was also granted by the clinical directorate of the ambulatory clinic at UOG-CSH. Verbal informed consent was obtained from HF patients or their caregivers after explaining the study's purpose. For the qualitative part, physicians provided informed consent via email or text. Privacy and confidentiality were maintained, and all records were password-protected and accessible only to authorized personnel.
