Ethics approval and consent to participate
: Ethical approval has been obtained locally from the ethics committees of both hospitals, the Westfriesgasthuis in Hoorn and the Flevoziekenhuis in Almere, and has been granted in both hospitals by the board of directors. Ethical approval has also been obtained and been granted by the provincial ethics committee, the <i>Medisch Ethische Toetsingscommissie Noord-Holland</i>, from the <i>Medisch Centrum Alkmaar</i>, in Alkmaar. The registration number is M016–034. For this type of study formal consent was not required.
