: Georgia: Use of these data was determined to be exempt from review as a public health surveillance activity and approved by the Georgia State University IRB (approval number H11142). California: The CA SCDC Program and this study were reviewed and approved by the California 82 Committee for the Protection of Human Subjects, the Public Health Institute IRB (approval number 15-10-2249), and the IRBs at 83 clinical sites reporting case data.
