Ethics approval and consent to participate
: All participants (patients) in the POPPI trial provided written informed consent. Ethical approval was granted by National Research Ethics Service Committee South Central - Oxford B (reference: 15/SC/0287).Ethical approval for the POPPI trial elicitation was given by the London School of Hygiene and Tropical Medicine observational research ethics committee.
