: Study protocols were reviewed and approved by the London School of Hygiene & Tropical Medicine Ethics Committee, the Ministry of Science and Technology in Ethiopia, the National Research Ethics Committee in Nigeria and the Gombe State Government in Nigeria. Written consent was obtained from all participants; illiterate participants were asked to appoint a proxy to sign on their behalf.
