: The proposal for this work was reviewed by the Officer of the Oxford University Medical Sciences Interdivisional Research Ethics Committee, with reference to formally approved process (Reference R61490/RE001). It was deemed to not require ethical review (18 December 2018) and hence they did not express a view on taking verbal consent. Our participants had already indicated their consent to participate by responding positively to the invitation. As noted in the Methods section (Methods, last para, page 6) of the paper, we elicited participant consent for the interview to be tape-recorded and transcribed verbatim.
