According to the UK Health Research Authority guidance ) service evaluations do not need ethical approval. We believe this sort of study comes under section 2.3.14 of the UK NHS Governance arrangements for research ethics committees: 2020 edition (GAfREC_Final_v2.1_July_2021_Final.pdf). Written informed consent was not obtained. The survey was entirely optional. Completion constituted consent to take part in the study, and no further consent forms were collected.
