: Ethics approval for the GUiNZ study was granted by the New Zealand Ministry of Health Northern Y Regional Ethics committee (NTY/08/06/055) (). Written informed consent was obtained from the children’s legal guardian(s) (from all participating mothers in 2009 and 2010) for enrolment into the GUiNZ study. Additional informed consent to link the routinely collected health records was obtained during child observation of 54 M DCW.
