: As described in the Methods section, “The design and conduct of the surveillance analyses were approved by both Southeastern Regional Medical Center (local) and Duke University Medical Center (oversight) Institutional Review Boards.” However, as the subject data was initially acquired as part of a quality assurance program and all patients were de-identified following completion of data acquisition, the need for consent to participate in this analysis was waived by both the Southeastern Regional Medical Center and Duke University Medical Center Institutional Review Boards.
