: Overall ethical approval was obtained from the Cardiff Multi-Centre Research Ethics Committee [08/MRE09/39] and local governance approvals were obtained from PCTs for each site. Governance approval was also obtained from Her Majesty’s Prison Service (HMPS), National Offender Management Service (NOMS) [NRC 22/10] and their Research Quality Assurance (RQA) process [RQA 518].
