: Ethical approval was obtained from Sunderland NHS Research Ethics Committee, Sunderland and North Tyneside CCGs and from Sunderland University Ethics Committee. Participants may have been vulnerable on a number of levels so an enhanced Criminal Records Bureau (CRB) was obtained by IG prior to data collection.
