Ethics approval and consent to participate
: A verbal consent was obtained from all patients (<i>n</i> = 943) in the GP-consultation. A written informed consent was obtained from all patients categorized into the high-risk group (<i>n</i> = 170). As mentioned in the Discussion section, the study initially was not meant to be a scientific one. However, the interventional rehabilitation programme included collection of a large amount of data and the interest grew to perform statistical analyses. Therefore, an application for ethical approval was sent to the Regional Ethics Review Board in Stockholm, Sweden, which approved the study including the consent procedures, Act number 2008/980–31/3.
