Ethics approval and consent to participate
: No ethical approval or obtainment of formal written informed consent was necessary for the study as PCPs were not subjected to procedures or required to follow rules of behaviour. This is in line with the Dutch guidelines, i.e. the Dutch Medical Research Involving Human Subjects Act []. Before the interviews, PCPs were given assurances about the confidentiality of their contribution, and were asked for verbal informed consent to participate in the study and audiotape their responses.
