Ethics approval and consent to participate
: Prior to inclusion, all participants had to sign a written informed consent form. Written informed consent was also obtained from a parent or guardian for all minor participants under the age of 18. The study was approved by the ethical committee of the Ludwig-Maximilians-Universität München (reference number 17-431) and have therefore been performed in accordance with the ethical standards laid down in the 1964 Declaration of Helsinki and its later amendments.
