: The CHANGE-3 study was led by the Institute of General Practice, University Medicine, Rostock. The evaluation studies concerning the regional intervention and practice team intervention were approved by the Ethics Committee in Rostock (A-2017-0162 and A-2017-0134). The study concerning the process evaluation was approved by the Ethics Committee of the University of Heidelberg (reference number S-349/2018). All participants gave their written informed consent to participate prior to data collection.
