: Information from 965 physicians and 999.364 patients registered in one health district were all anonymized and the Rio de Janeiro Health Department (responsible for the safekeeping of the data) provided the consent to use it in research. All information was anonymized, making it impossible for patients, doctors and clinics to be identified from this manuscript. The study was conducted in accordance with the 466/12 resolution from the Brazilian National Health Council and the Declaration of Helsinki and it was approved by the Rio de Janeiro Municipal Health Department (RJ-MHD) research ethics board.
