: The OPEM programme was approved by the Research Ethics Board at Sunnybrook Health Sciences Centre and the Women’s College Health Sciences Centre in Toronto, Ontario.<sup>4</sup> Because all study outcomes were measured using ICES data, informed consent from the physicians and the patients was not required. We sought and received approval from the College of Physicians and Surgeons of Ontario to link their data on physician identifiers to the administrative databases held at ICES.
