: This research was done without patient involvement. The unique link to the survey started by a brief description of the study objectives, survey content and privacy safeguards. Participants were subsequently presented with a waiver and informed consent, before starting the questionnaire. The study was submitted to the Norwegian Centre for Research Data (NSD) and the Central Norway Regional Committee for Medical and Health Research Ethics where no further approval was required (Reference 688223/19). This study was conducted in accordance with the Declaration of Helsinki and Norwegian regulations. Informed consent was obtained from all the participants in this study.
