: An application to the Ethical Review Authority (EPM) was not required according to Swedish legislation about ethical review of research involving humans (3-4 §§, Lag [2003:460], since the survey was answered anonymously and did not contain sensitive personal data. All methods were carried out in accordance with Swedish guidelines and regulations. EPM confirmed in an advisory statement that an application to ethical review committee was not necessary according to Swedish legislation.The survey included information indicating that participation was voluntary and that the responses were collected anonymously. A survey response by the physicians was considered as consent to participate, thus consent was obtained. According to the Swedish Ethical Review Authority, the need to obtain other forms of consent from participants was not necessary as the participants answered voluntarily and anonymously.
