Ethics approval was granted by the Liverpool John Moores University Research Ethics Committee (UREC reference: 16/NSP/035). Written informed consent was obtained from all subjects prior to participation. Parents provided written or verbal consent on behalf of their children (aged under 16), while the children gave verbal consent for the interview, nurse visit and measurements.
