: The study was approved by NOVA Medical School (NMS) Ethics Committee (18/2023/CEFCM), before data collection began. All participants were provided with an informed consent form before the interview. For the documentary analysis component, we only analysed publicly available documents. All study procedures were performed in accordance with relevant laws and institutional guidelines.

: BH is a Portuguese Family Physician working in a FHU with the pay-for-performance scheme described in this paper. He was a member of the Technical Group for the Review of Indicators for Monitoring Personal and Family Health Performance, a committee later replaced by the National Technical Committee (NTC) discussed in this paper.
