: Ethics for this study was obtained from the University of Alberta Health Research Ethics Board (HREB) (Pro00130498). This research involving secondary data use of healthcare administrative data was done in accordance with the declaration of Helsinki. The use of data was legally applied for and agreed to by the provincial data custodians.
