: The study did not require approval from a research ethics committee. This assessment was confirmed by the Regional Committee on Health Research Ethics in the North Denmark Region, in accordance with Danish legislation (Act on Research Ethics Review of Health Research Projects, Consolidation Act no. 1338 of 1 September 2020). The study was conducted in accordance with the ethical principles of the Declaration of Helsinki.The study involved qualitative interview data from healthcare contexts. No access to health registries was involved, and data were handled in accordance with applicable data protection regulations. Data protection and GDPR compliance were reviewed by the institutional data protection officer at Aalborg University.Audio recordings and transcripts were stored on secure, password-protected university servers with restricted access. Audio recordings were deleted after transcription. All transcripts were pseudonymised during transcription, and identifying information was removed prior to analysis. Data will be stored for five years in accordance with institutional data management guidelines and then securely deleted.In addition to the qualitative interview and workshop data, the study draws on aggregated descriptive data from more than 150 APN-supported care trajectories to provide contextual background for the study setting. These data originate from routinely compiled municipal documentation related to the implementation and ongoing development of the APN function.The aggregated data were accessed and reported only in anonymised form and were not collected specifically for research purposes. They were used solely to describe the overall characteristics of the population served by the APN model and were not subjected to analytical procedures or linked to individual participants.No individual-level patient data or health registry data were accessed for the purposes of this study. The use of aggregated municipal data for publication was assessed and handled in accordance with applicable data protection legislation and municipal data governance procedures.Participation was voluntary, and all participants received oral and written information about the study. Written informed consent was obtained prior to participation. Participants were informed of their right to withdraw at any time without consequences, and that their data would be deleted upon withdrawal if requested.Special attention was given to participants considered potentially vulnerable, including frail older adults and individuals with mental health or substance use challenges. Interviews were conducted with sensitivity to participants’ capacity and well-being, and participants could pause or terminate interviews at any time.
