: According to Danish law (i.e. the Act on Research Ethics Review of Health Research Projects, Act number 593 of 14 July 2011, Sect. 14(2)) notification of questionnaire surveys and medical database research projects to the system of research ethics committee system is only required if the project involves human biological material. Thus, approval from the Committee on Health Research Ethics was not required as no biomedical intervention was performed in this study. The study was approved by the Danish Patient Safety Authority (reference number: 3- 3013- 1274/1).
