: The study was performed in accordance with the 1975 Declaration of Helsinki (as revised in 1983), and the protocol was approved by the ethics review committee at Osaka Medical College (No. 730) and was registered in the University hospital Medical Information Network Clinical Trial Registry (UMIN000033984).
