Ethical approval was obtained from the Medical Research and Ethics Committee of Universiti Malaysia Sarawak (UNIMAS/TNC(PI)/09–65/01 Jld.3(90)). Informed written consent was obtained from all individual participants prior to data collection. For participants with limited literacy, the study objectives and procedures were explained in their preferred local language, and consent was witnessed and documented via thumbprint in accordance with institutional ethical guidelines.
