: This study was conducted in accordance with the ethical principles outlined in the Declaration of Helsinki. The departmental committee approved the study protocol before case collection. The study protocol was discussed with all the patients and those accepted to participate were asked to sign fully informed oral consent before enrolment. After complete case collection, the approval by the Research Ethics Committee of the Faculty of Medicine, Ain Shams University was obtained.
