Ethics approval and consent to participate
: The procedure for obtaining informed consent was developed to comply with capacity legislation governing England and Wales (Mental Capacity Act 2005, Sections 30–34). Informed consent was obtained from participants with the capacity to consent. If they did not have capacity, a personal or professional consultee was asked to give agreement for the person’s participation, and sign his/her agreement for this. Written consent was obtained from all participants. The study was reviewed and approved by the London Queen Square Research Ethical Committee (17/LO/0430) and the UK Health Research Authority.
